  • Your application is valid for one year from the date you submit it. After that, it is removed from the system and the fee is non-refundable — so make sure to complete the process within that window
  • The total cost is $65 application fee + $5 portal fee (non-refundable)
  • After submitting, you’ll receive a Locator ID / Authorization Code (an 8-digit number) — save this! You’ll need it when you schedule your in-person appointment
